LumiLife VIP Membership Terms & Policies
Welcome to the LumiLife VIP Membership, designed to reward patients who are committed to maintaining healthy skin and natural-looking aesthetic results through physician-directed care.
Membership provides exclusive pricing, special benefits, and priority access to select services. By enrolling, you agree to the following terms and conditions.
1. Monthly Membership
The LumiLife VIP Membership is billed automatically each month using the payment method on file.
Membership provides access to exclusive member pricing and benefits. It is not a prepaid treatment plan, savings account, or stored-value program.
Monthly membership fees:
- Are non-refundable
- Do not accumulate as treatment credit
- Have no cash value
- Cannot be redeemed for products or services
2. Membership Commitment
Enrollment requires a minimum six (6) month commitment.
Membership cannot be canceled or paused during the initial commitment period.
After the initial six months, your membership automatically renews on a month-to-month basis until canceled.
To cancel, written notice must be received at least 30 days before your next scheduled billing date.
Membership benefits remain active through the final paid billing cycle.
3. Membership Benefits
Active members enjoy exclusive benefits, which may include:
- Preferred pricing on eligible treatments
- Savings on Signature Treatment Programs
- Savings on injectables and regenerative therapies
- Complimentary annual ClearView® Skin Analysis
- Birthday treatment benefit
- Priority appointment scheduling
- Invitations to exclusive member events and promotions
Membership benefits apply only while the membership is active and in good standing.
Benefits may not be transferred to another person.
4. Member Pricing
Member pricing is available only to active VIP Members.
Member discounts:
- Apply only to regular-priced services.
- Cannot be combined with promotional offers, package pricing, flash sales, referral rewards, gift certificates, or other discounts unless specifically stated.
- Apply only to the enrolled member.
Botox® pricing, specialty products, and selected treatments may be excluded from member pricing and will be discussed during your consultation.
5. Complimentary Benefits
Complimentary membership benefits, including birthday treatments and annual skin analyses, are available only after completion of the initial six month membership commitment and while the membership remains active.
Unused complimentary benefits:
- Do not roll over.
- Have no cash value.
- Cannot be exchanged for other services or products.
6. Payment Policy
A valid credit or debit card must remain on file throughout your membership.
Monthly dues are automatically charged on your scheduled billing date.
If payment is declined, membership benefits may be suspended until payment is received.
Membership may be canceled if payment remains outstanding for more than 30 days.
7. Scheduling & Appointment Policies
VIP Members receive priority scheduling whenever possible.
Appointment availability is based on provider schedules and cannot be guaranteed.
All appointments remain subject to LumiLife’s Appointment; Cancellation Policy.
Late cancellations, no-shows, and package forfeiture policies apply equally to members.
8. Medical Eligibility
Membership does not guarantee treatment.
All procedures require physician evaluation and medical clearance.
Treatment recommendations are based on your medical history, examination findings,
safety considerations, and individual goals.
LumiLife Aesthetics + Wellness reserves the right to decline or postpone treatment whenever it is determined to be medically inappropriate.
9. Membership Changes
LumiLife Aesthetics + Wellness reserves the right to modify membership pricing, benefits, or terms.
Members will receive advance notice before any changes become effective.
Membership enrollment may be limited or temporarily closed to maintain appointment availability and quality of care.
10. Membership Is Non-Transferable
Membership benefits belong exclusively to the enrolled member.
Memberships, discounts, complimentary treatments, and promotional benefits:
- Cannot be transferred.
- Cannot be shared.
- Cannot be exchanged for cash.
- Cannot be applied to another persons treatment.
11. Physician Treatment Programs
Patients enrolled in physician-directed treatment programs (such as Hair Restoration Programs or Skin Renewal Programs) receive discounted pricing based on completion of the recommended treatment series.
If a treatment program is discontinued before completion, any complimentary treatments included within the program are forfeited, and previously completed treatments will be recalculated at the standard individual treatment price.
This policy allows us to offer significant savings to patients who complete their recommended treatment plans.
12. Practice Rights
LumiLife Aesthetics + Wellness reserves the right to suspend or terminate membership for:
- Failure to maintain a valid payment method.
- Repeated missed appointments or late cancellations.
- Failure to comply with office policies.
- Abusive, disruptive, or inappropriate behavior toward staff or other patients.
- Failure to pay applicable fees.
Membership termination does not entitle the member to a refund of previously paid membership fees.
13. Agreement
By enrolling in the LumiLife VIP Membership, you acknowledge that you have read, understood, and agree to these Membership Terms &; Policies.
You authorize LumiLife Aesthetics + Wellness to automatically charge the payment
method on file for monthly membership dues and any applicable fees authorized under our Appointment &; Office Policies.
